Nomination Package Checklist For NDU Schools

Use this checklist as a guide for completing and assembling the nomination package. Forward the checklist with the package for the convenience of MACOM/ equivalent endorsers/reviewers to aid in ensuring that the package is complete. The "on line" forms provided at the links below must be used. The applicant is responsible for obtaining and assembling all documents with the exception of item 5 which will be added by the appropriate representative at MACOM Headquarters or equivalent level. Make a checkmark by each item that is attached. Items 7, 8, 9 are applicable only to individuals who are applying for the Senior Acquisition Course (SAC) at ICAF - nominee will indicate "N/A" in lieu of a checkmark if not applying for this program. Documents must be assembled in the order listed.

3. ___ Resume of career history, and significant education and training. Follow format at this link (One-page Limit, no-exceptions) - Must be attached

4. ___ Two Letters of recommendation (1 from supervisor, 1 from any other individual in chain of command) - Must be attached

5. ___ MACOM/equivalent endorsement reflecting rank order of all nominees from that MACOM or equivalent organization - Must be attached.

6. ___ Nominee Statement of Career Aspirations, Goals, and Suitability for the Program (Two-page limit, including signatures) - online

7. ___ ICAF Senior Acquisition Course Nomination Form (ICAF SAC applicants only)("Agency Representative" must be applicant's supervisor) - online

8. ___ Senior Rater Potential Evaluation (ICAF SAC applicants only) - Must be attached

9. ___ Acquisition Career Record Brief (ICAF SAC applicants only) - Must be attached

10. ___ One official transcript for highest degree earned. DO NOT unseal the envelope from the institution. - Must be attached

11. ___ Proof of Army Training Prerequisite attached (copy of CES AC course certificate or signed CES Approval Memo for Army Civilians; copy of NSF course certificate or signed DOD Waiver memo for DLAMP applicants)

12. ___ NDU Student World-Wide Travel Form ("Agency Representative" must be applicant's supervisor)- online

13. ___ NDU Authorization to Release Education Records - online   (Updated for FY07)

14. ___ Agreement to Continue in Service - online

15. ___ SSC GPP Mobility Agreement - online
